SECTION 1	SURNAME	Exactly as it appears in your passport
SECTION 2	FULL NAME	First name(s) only as it appears in your passport
SECTION 3	OTHER NAMES OR SURNAMES USED IN THE PAST	This would pertain to those that had a name change including women through marriage.
SECTION 4	DATE OF BIRTH (Day/Month/Year) COUNTRY and PLACE OF BIRTH
SECTION 5	SEX	Check the correct box
SECTION 6	NATIONALITY	Country of citizenship
SECTION 7	PERSONAL NUMBER	Here you can use a Social Insurance Number (Social Security Number in the USA) or Drivers license number.
SECTION 8	ADDRESS OF PERMANENT RESIDENCE	List your full mailing address.
SECTION 9	PASSPORT DETAILS :
	TYPE	Normally the letter "P" - you’ll find this on the top left hand corner of your passport’s photo page;
	NUMBER	Copy your number exactly as it appears in your passport;
	DATE OF ISSUE & VALID UNTIL	Copy these exactly as they appear in your passport;
	ISSUING AUTHORITY	You should locate this information on the bottom right hand corner of your passport's photo page.
SECTION 10	MARITAL STATUS	Check the correct box
SECTION 11	INFECTIOUS DISEASES	Check the correct box
SECTION 12	CRIMINAL OFFENCES	Mark yes and then specify - ONLY if you have ever been charged with a serious offence. Do not list traffic or minor offences
SECTION 13	HAVE YOU EVER BEEN LIMITED OR PROHIBITED FROM AN ENTRY TO UKRAINE	If yes, then explain, otherwise check "no"
SECTION 14	HAVE YOU EVER BEEN DEPORTED OR REMOVED FROM UKRAINE	Check the correct box
SECTION 15	PURPOSE OF YOUR JOURNEY	For private Visa write "visiting family & friends" or "business" or "visitor" for Tourist Visa you require an official invitation, which we can supply.
SECTION 16	DURATION OF STAY IN UKRAINE	Must be 6 months or less for "Private/visitor or Tourist" visa.
SECTION 17	DATE OF PROPOSED ENTRY TO UKRAINE	Be sure to state an exact date that is clearly in advance of the date you actually plan to arrive in Ukraine. DO NOT state a range of dates. Your visa will not be valid until this date, so it is imperative that you indicate a date that is earlier than your proposed date of travel.
SECTION 18	POINT OF ENTRY TO UKRAINE	Write the name of the first city you will arrive in. (Kyiv, Lviv, etc.)
SECTION 19	MEANS OF TRANSPORT FOR ENTRY TO UKRAINE	Depending on your means of transport e.g. Airline (state name), Rail, Bus etc.
SECTION 20	NAME AND ADDRESS OF ORGANIZATION WHICH INVITES	It is only necessary to fill in this section if you are traveling as a tourist as this is the only category that still requires an invitation.
	NAME AND ADDRESS OF PRIVATE PERSON WHO INVITES	No longer required to fill in this section if you are traveling on a Private visa.
SECTION 21	CITIES IN UKRAINE YOU INTEND TO VISIT	State the main cities you will be visiting. This information does NOT limit your travels once in the country. Present laws allow you to travel wherever you wish.
SECTION 22	ADDRESS OF TEMPORARY ADDRESS IN UKRAINE	Hotel or friend’s name and address.
SECTION 23	IN CASE OF NEED WHO WILL GIVE FINANCIAL SUPPORT	Usually you would simply write "self" or if on business then the company you work for or the company that invited you.
SECTION 24	CHILDREN UNDER 16 YEARS INCLUDED ON YOUR PASSPORT AND WHO WILL TRAVEL WITH YOU TO UKRAINE.	Only list this information if your children are traveling with you.
SECTION 25	IF YOU HAVE BEEN TO UKRAINE	If so list date, if not leave blank.
SECTION 26	VISA REQUESTED FOR	Check the appropriate box
SECTIONS 27-31		Complete only if you are seeking a transit visa through Ukraine.

Sign and date the application form and it is ready to be submitted to your nearest Ukraine Consulate together with your passport, two passport-type photos and the required fee.

As a note of interest – A "Tourist" Visa is what most travelers would normally request, however Ukraine continues to create barriers to tourism by requiring that all applications for Tourist Visas be accompanied by a formal invitation, which we can supply if necessary.
